Help with mobility like transfers, walking, using assistive devices, and completing exercises per physical therapy request. Provided skin care and observe of any pressure areas. I would assist with household duties such as laundry, dishes, bed linen, and learn the patients cleaning routine. Meal planning, preparation and grocery shopping or errands. I would check and record vital signs directed by the care plan. Observe and document and change and report any safety concerns. Companionship and daily living support such as providing social interaction, assist with scheduling, daily routine, and safe support in activities. Other tasks were medication reminders and assisting with other programs under nurse or therapist direction
